Applicant's Statement


In connection with my application for volunteer service with The Chapel, I authorize The Chapel to solicit background information relative to my criminal record history. I understand that The Chapel may conduct inquiries into my background that may include criminal records, personal references, and other public record reports pertaining to me.

I release The Chapel, their respective employees, or SEARCHLINK, their agent and employees, and all persons, agencies, and entities providing information or reports about me from any and all liability arising out of furnishing any such information and reports.

Should my application be accepted, I agree to refrain from unscriptural conduct in the performance of my services on behalf of The Chapel. I also agree to attend any required meetings which will aid in the further development of Chapel Ministries. I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE AS MY OWN FREE ACT.
