Resources for Deacons
Below is information that will help you to connect with people and resources from our Care and Benevolence Teams, should you need to use them in a ministry situation.
Biblical Counseling, Support Groups, and Visitation:
If you are working with someone who would benefit from free, biblical counseling, you can send them this link: https://thechapel.com/care-support/ (Care & Support | The Chapel)
If they would like biblical counseling, they would need to click on the biblical counseling tab. From there, they would choose a PDI Form to complete (this stands for Personal Data Inventory Form). There are different PDI’s for different age groups. Once they fill out the PDI and submit it, our counseling ministry will contact them to arrange an initial appointment.
Important to know:
- If they are applying for pre-engagement, pre-marital, or marriage counseling, each party in the relationship needs to fill out their own PDI.
- While they are in counseling, they need to be attending The Chapel on Sunday mornings, or listening to the sermons at the very least.
This link will also provide them with information for our support groups. We offer support groups for folks seeking help with issues of both sin and suffering. This page provides them with a brief description of the group, and contact information needed for next steps.
This page also provides them with access to the Potter’s Hands Webpage, should you or they need to request support from the Potter’s Hands ministry.
This page also provides information related to visitation, if they would like someone from our visitation team to spend time with them.
Potter’s Hands
Potters Hands is “a ministry specifically centered around meeting the tangible needs of families in Western New York. With our warehouse of gently used furniture and household items, Potter’s Hands helps those that need it most. To do this, we partner with local agencies and churches all over our region. Our desire is to be the hands and feet of Jesus by loving and assisting our neighbors.” If someone at your campus needs assistance from Potter’s Hands, they can go to https://thechapel.com/next-steps/the-potters-hands/ (The Potter's Hands | The Chapel) in order to take the next steps.
Please note: The Needs request list needs to be filled out by a caseworker. A church member, pastor, deacon, or other Chapel staff can fill out the needs request list. It cannot be filled out directly by the person in need as it the responsibility of the person filling out the form to vet the need. The current procedure is for the person who submits the form to then accompany the client to the warehouse to select the needed items.
Congregational Care
Our Congregational Care Director is currently Allison Granger. She oversees many things that our church offers to support others. This includes weddings, funerals, ParentBaby Dedications, sending cards or flowers to those who could use the encouragement, or meals to those who need it. Benevolence
At The Chapel, we have a benevolence ministry. This benevolence ministry seeks to provide short-term financial and material assistance when possible to those in need. While this ministry helps people outside of our church, it will prioritize those actively participating in the life of our church. If you are working with someone going through a financial hardship who could benefit from short-term help, you can send to them this link - https://thechapel.churchcenter.com/people/forms/186850 (Benevolence Review Form - The Chapel). This is a form that they can fill out directly, please do not fill out on their behalf. Once completed, this form will go directly to Allen Hammann, who oversees the ministry. Uber
We offer Uber rides to those who wish to come to church, yet do not have a ride. This is a resource that The Chapel pays for. If you come in contact with someone who needs an Uber ride for a Sunday morning, please utilize the contact information below for the appropriate campus.

Safety
If you are working with an individual who you believe is either the cause or the victim of a potentially abusive situation, please inform the Safety Team here: send a message. In your email to them, please provide as many details as possible.
